  • Able to attend any of the Millard Health meetings scheduled to review your progress.

  • Able to ask questions and be part of the decisions that are made about you.

  • Able to refuse services at any time during your program.

  • Recommended to a more appropriate health care provider if you need treatment that is not available at Millard Health.

  • Able to file a complaint, or appeal a decision made concerning your program, and receive a written or verbal response - knowing your care will not be affected.

  • Aware that your information is kept private and shared only as appropriate with organization that referred you to Millard Health (WCB, lawyer, insurer), your employer, and your family physician.
