Any medical or work related information you can provide on your client's condition (medical information from doctors/specialists, test results, a Physical Demands Analysis or job descriptions) will help our Admissions Coordinator to proceed with your referral.

Note: In order to serve our customers better, a follow -up telephone survey will be completed (6) months post-discharge by an independent research group. Our Customers may be contacted to obtain feedback in regards to our services. Participation is voluntary and the feedback is anonymous.
